First launch - premature epoxy fail

Previously pressure tested to 80psi.
But on very first launch, at around 50psi the cap detached from the epoxy in the hose coupling.

I'm thinking that previous screwing and unscrewing had weakened the bond.
Previously I heard mention of hose couplings with an internal thread to fit a bottle.
Are these available, and if so where from?
